docs: update stale version references in internal/version.go comments
- Fix Version History 'Current' to match actual constant (v0.4.1 -> v0.4.2)
- Correct usage comment to reflect actual single use (gt version subcommand)
- Update SemVer example, ldflags example, and example output to v0.4.2
