refactor(lsp,llm,hexailsp,appconfig): split long funcs; add tests
- Extract helpers to keep funcs <=50 lines; no behavior changes
- Add tests for prompt removal, code actions, and LLM request builders
- Table-drive TestInParamList; run gofmt
