fix: pass RequestTimeout to all llm.Config instances
Add RequestTimeout field to llm.Config construction in:
- internal/hexailsp/run.go (buildClientIfNil)
- internal/llmutils/client.go (NewClientFromApp)
- internal/lsp/server.go (newClientForProvider)
This ensures the configured timeout is used in all code paths,
not just the ones already updated.
