cli: stream responses in hexai when supported (OpenAI, Ollama)
- Add llm.Streamer optional interface
- Implement ChatStream for OpenAI (SSE) and Ollama (JSON stream)
- CLI uses streaming; LSP unchanged (non-streaming)
- README: document streaming behavior for CLI
