fix: populate stub reference files, remove redundant TL;DR, fix HTML entities
- Populate 19 stub files with actual content from 100go.co
- Remove redundant bare 'TL;DR' line after '#### TL;DR' header in 76 files
- Convert 3 mid-file '#### TL;DR / Note' sections to '#### Note'
- Fix HTML entities (< > &) to proper characters in 9 files
